Dental fillings: what you need to know

A cavity never heals on its own. Bacteria dissolve the enamel, then the dentin underneath, and the closer they get to the nerve, the more complex the treatment becomes. A filling done early stops the process, protects the tooth and avoids a root canal or a crown later on.

How the treatment works, step by step

1. Diagnosis

The dentist confirms the cavity with the exam and an X-ray, checks how deep it is and makes sure the nerve is not affected.

2. Local anesthesia

A topical gel numbs the gum, then a local anesthetic makes the tooth completely numb. For very small cavities, anesthesia is sometimes unnecessary.

3. Removing the decay

The decayed tissue is removed with a small rotary instrument, keeping as much healthy tooth as possible.

4. Preparation and bonding

The cavity is cleaned, the enamel is etched and a bonding agent is applied so the composite adheres firmly.

5. Placing the composite

The resin is placed in thin layers, each hardened with a blue light, and sculpted to restore the natural shape of the tooth.

6. Adjustment and polishing

The bite is checked and the filling is polished so it feels smooth and blends with the surrounding tooth.

Alternatives and options

When the cavity is very large or the tooth is weakened, an inlay/onlay or a crown protects the tooth better than a large filling. If the decay has reached the nerve, a root canal treatment is required before restoring the tooth. Silver amalgam fillings remain an option in some cases, but composite is our first choice for its appearance and conservative preparation.

Frequently asked questions about dental filling

Does a white filling last as long as a silver one?

Modern composite fillings last many years, comparable to amalgam in small and medium cavities, provided oral hygiene is good. For very large fillings on molars, the dentist may recommend a crown or an onlay instead.

Is getting a filling painful?

No. The tooth is numbed with local anesthesia. You may feel pressure or vibration, but no pain. Mild sensitivity in the days that follow is normal.

How much does a filling cost?

The cost depends on the size of the filling and the number of surfaces involved. You receive a written estimate before treatment; most insurance plans and the CDCP cover fillings, and the RAMQ covers them for children under 10.

Should all my old amalgam fillings be replaced?

Not automatically. An amalgam filling in good condition can stay in place. It is replaced when it is broken, leaking or when decay develops underneath, or on request for cosmetic reasons after discussion with your dentist.

Can I eat after a filling?

Yes, once the numbness has worn off, usually within one to three hours. The composite is fully hardened when you leave the clinic.

Why is my tooth sensitive after a filling?

Sensitivity to cold or pressure for a few days to a few weeks is common, especially after a deep filling. If the pain increases or lasts longer, come back for a check: the bite may need adjusting.
