Tooth extraction: what you need to know

Saving a tooth is always our first goal. But when a tooth is too damaged to be restored, when its roots are fractured or when an infection keeps coming back, extraction becomes the treatment that protects your health and relieves the pain. Done under local anesthesia with modern instruments, it is quick and far more comfortable than most people expect.

At Go Dentiste, our dentists perform simple and surgical extractions at both clinics, including for emergencies. Before the procedure, we discuss how to replace the tooth if needed, whether with an implant, a bridge or a partial denture, so that the gap does not cause the neighbouring teeth to shift.

Common reasons for extracting a tooth

  • A tooth destroyed by decay, with too little structure left to hold a filling or a crown
  • A vertical crack or a fracture that extends below the gumline
  • Advanced periodontitis with a tooth that has become loose
  • An infection or abscess that cannot be resolved with a root canal
  • A primary tooth that does not fall out on its own and blocks the permanent tooth
  • Crowding, on the recommendation of an orthodontist

How the treatment works, step by step

1. Assessment and X-ray

The dentist reviews your health history and medications, examines the tooth and takes an X-ray to see the roots and the surrounding bone.

2. Local anesthesia

The area is numbed completely. You feel pressure during the extraction, but no pain.

3. Loosening and removing the tooth

Using an elevator and forceps, the dentist gently loosens the tooth from its socket. A surgical extraction may require a small incision or dividing the tooth into sections.

4. Cleaning the socket

Any infected tissue is removed and the socket is rinsed. A bone graft can be placed at this stage if an implant is planned.

5. Stitches and gauze

Stitches are placed when needed, and you bite on gauze for 30 to 45 minutes to form a stable blood clot.

6. Instructions and follow-up

You leave with written instructions and, if necessary, prescriptions. A follow-up visit checks the healing and removes stitches.

After the treatment: advice

  • Bite on the gauze for 30 to 45 minutes and avoid rinsing, spitting or using a straw for 24 hours
  • No smoking for at least 72 hours: it is the main cause of dry socket
  • Apply ice on the cheek for 20 minutes at a time during the first day to limit swelling
  • Eat soft, lukewarm food and chew on the other side for a few days
  • From the next day, rinse gently with warm salt water after meals and keep brushing the other teeth

Alternatives and options

When the tooth can be saved, a root canal treatment with a crown is the alternative to extraction, and it is our first recommendation whenever it is realistic. For a loose tooth caused by gum disease, periodontal treatment can sometimes stabilize it. After an extraction, the tooth can be replaced with a dental implant, a bridge or a removable partial denture.

Frequently asked questions about tooth extraction

Does a tooth extraction hurt?

No, the tooth is fully numbed. You feel pushing and pressure, not pain. Afterwards, discomfort is usually mild and controlled with ibuprofen or acetaminophen for two to three days.

How long does it take to heal after an extraction?

The gum closes in 7 to 14 days. Complete bone healing takes three to six months, which is why an implant is usually placed a few months later, unless immediate placement is possible.

What is dry socket and how do I avoid it?

Dry socket (alveolar osteitis) happens when the blood clot is lost too early, exposing the bone and causing sharp pain a few days after the extraction. Avoid smoking, straws, spitting and vigorous rinsing for the first days. If it happens, a dressing placed by the dentist relieves the pain quickly.

Can I drive after an extraction?

Yes, with local anesthesia alone you can drive yourself home. If you have taken a sedative medication, you will need someone to drive you.

Do I need to replace the extracted tooth?

For back teeth and visible teeth, yes in most cases: an empty space lets the neighbouring teeth tilt and the opposing tooth over-erupt, and the bone shrinks over time. Wisdom teeth and some second molars are exceptions.

Extraction or root canal: how do we decide?

It depends on how much healthy tooth remains, the condition of the root and the bone, and the cost of each option over the long term. Your dentist shows you the X-ray, explains both scenarios and gives you a written estimate for each.
