Wisdom teeth removal is the extraction of the third molars, which erupt between ages 17 and 25 and often lack the room to come in properly. It is recommended when they are impacted, partially erupted, decayed, infected or pushing on the neighbouring teeth.

Wisdom teeth removal: what you need to know

Wisdom teeth are the last to appear, at the back of the mouth, at an age when the jaw has finished growing. In many people there is simply not enough space: the teeth stay stuck in the bone, come in sideways or erupt only partway, creating a pocket where food and bacteria collect. Pain, swelling, repeated infections and cavities on the second molars are the usual consequences.

At Go Dentiste, the assessment starts with a panoramic X-ray that shows the position of the roots in relation to the nerve and the sinus. Most extractions are done at our Valleyfield and Vaudreuil-Dorion clinics under local anesthesia, with nitrous oxide available for anxious patients; the most complex cases are referred to an oral surgeon.

When should wisdom teeth be removed?

  • Pain, pressure or swelling at the back of the jaw
  • Repeated gum infections around a partially erupted tooth (pericoronitis)
  • A cavity on the wisdom tooth or on the second molar next to it that cannot be cleaned
  • A tooth that is impacted or tilted against the neighbouring tooth on the X-ray
  • A cyst or bone loss around the tooth
  • Before or after orthodontic treatment, on the orthodontist’s recommendation

How the treatment works, step by step

1. Assessment with a panoramic X-ray

The dentist examines the position of each wisdom tooth, its roots and its relation to the nerve of the lower jaw and the sinus above.

2. Planning

You receive a written estimate, decide whether to remove one, two or all four teeth, and get instructions for the day of the procedure.

3. Local anesthesia

The area is fully numbed; nitrous oxide can be added for relaxation if you wish.

4. Extraction

For an erupted tooth, the removal is similar to any extraction. For an impacted tooth, the gum is opened and the tooth is sometimes divided into sections to remove it gently.

5. Stitches

Dissolvable stitches close the gum, and gauze is placed to control bleeding.

6. Follow-up

You leave with written instructions and prescriptions; a check-up is scheduled about a week later.

Benefits

  • Prevents repeated infections and pain at the back of the mouth
  • Protects the second molars from decay and bone loss
  • Easier and faster recovery when done at a young age
  • Avoids damage to orthodontic results in some cases
  • Done under local anesthesia at the clinic, with a single recovery period for all teeth

After the treatment: advice

  • Rest for the remainder of the day and keep your head elevated
  • Ice on the cheeks for 20 minutes at a time during the first 24 hours
  • No smoking, straws, spitting or vigorous rinsing for three days
  • Soft, cold or lukewarm food for the first days; avoid seeds and crunchy foods for a week
  • Take the prescribed medication as directed and call us if pain increases after day three

Alternatives and options

Wisdom teeth that have erupted fully, are well aligned, easy to clean and free of decay can be kept and monitored at each check-up. When only the gum over a partially erupted tooth is inflamed, a local cleaning and, occasionally, removal of the gum flap can buy time. Extraction remains the definitive solution for impacted or repeatedly infected teeth.

Frequently asked questions about wisdom teeth removal

At what age should wisdom teeth be removed?

Between 17 and 25 is generally ideal: the roots are not fully formed, the bone is more flexible and healing is faster. Removal is possible at any age, but recovery tends to be longer after 30.

Do all four wisdom teeth have to come out at once?

Not necessarily. Removing all four in one visit means a single recovery period, but the dentist can also remove one side at a time or only the problematic teeth, depending on your preference and your X-ray.

How many days off should I plan?

Two to three days for most patients. Swelling peaks around the second or third day, then decreases. Physical work and sports should wait about a week.

Is wisdom teeth removal covered by insurance?

Most private dental plans and the Canadian Dental Care Plan cover extractions, including surgical ones. We send a pre-determination to your insurer if you want to know the exact coverage in advance.

Can wisdom teeth push my front teeth out of alignment?

The evidence is mixed: crowding of the front teeth happens with age even in people without wisdom teeth. Wisdom teeth are removed for infection, decay, pain or damage to the neighbouring molar, not solely to prevent crowding.

What should I do if I have an infection around a wisdom tooth?

Call us the same day. The dentist cleans the area, may prescribe antibiotics if the infection is spreading, and schedules the extraction once the acute phase has settled or, when possible, immediately.
