01In brief
Baby tooth lost too early: is a space maintainer needed?
Worth assessing after the tooth is lost
When a baby tooth, especially a molar, is lost or removed well before the permanent tooth is ready to come in, the neighbouring teeth can drift into the space and block it. A space maintainer, a small appliance, holds that space until the permanent tooth arrives. It isn’t always needed: it depends on the tooth lost, the child’s age and when the permanent tooth is expected to come in. Evaluation in Valleyfield or Vaudreuil-Dorion: 514-533-3368.

02What to know
What to know
A space maintainer helps prevent a lack of space that would take longer to correct later.
- Baby molars matterThey hold the place of the premolars until around ages 10 to 12. Their early loss is the situation where a space maintainer is most often discussed.
- Front teeth, less oftenThe early loss of a baby front tooth rarely causes significant space loss; it’s discussed depending on the situation.
- X-rays guide the decisionThey show how far along the permanent tooth is. If it is about to come in, a space maintainer often isn’t needed.
- Regular follow-upThe appliance is checked at exams, then removed when the permanent tooth starts to come in.
When to mention it to the dentist
- A baby molar was removed because of a cavity or an abscess
- A baby tooth came out after a fall
- A baby tooth comes out much earlier than the one on the other side
- The neighbouring teeth seem to be moving closer together
- Your child’s appliance has come loose or broken
Good to know: A space maintainer that has come loose or broken is usually not an emergency, but it should be checked soon: keep it and call the clinic.

03Options
Three possible decisions
The dentist explains which one suits your child.
- No applianceThe permanent tooth is about to come in, or the lost tooth is unlikely to cause space loss: we monitor.
- Space maintainerMost often a band on a neighbouring tooth and a wire that holds the space.
- Orthodontic evaluationIf space has already been partly lost or other alignment problems are present, a full orthodontic evaluation is recommended.
04At our clinics
Follow-up at our clinics
After a baby tooth is lost or extracted, the dentist assesses the space and, if needed, takes an X-ray to locate the permanent tooth. The dentist explains whether a space maintainer would help, and how it would be placed and followed.
For children under 10, the exam and many dental services are covered by RAMQ (Quebec’s public health insurance plan); ask the clinic what applies to a space maintainer in your situation.

06FAQ
Frequently asked questions: space maintainer
Is a space maintainer always needed?
No. It depends on the tooth lost, the child’s age and when the permanent tooth is due to come in. An X-ray helps decide.
How long is it worn?
Until the permanent tooth starts to come in, which can range from a few months to a few years. It is checked at exams.
Is it uncomfortable for the child?
Most children get used to it within a few days. Very sticky foods, which can loosen it, should be avoided.
What should I do if it comes loose?
Keep it and call the clinic. It’s usually not an emergency, but the space needs to be protected soon.
What if nothing is done?
The neighbouring teeth can drift into the space and block the permanent tooth, which may call for longer orthodontic treatment later.
