01In brief

Impacted canine: what to do when a tooth doesn’t come in?

Screening around ages 10 to 12

An impacted canine is a permanent canine, most often an upper one, that stays stuck in the bone instead of coming in. It can damage the roots of the neighbouring teeth. Caught early, around ages 10 to 12, the situation can sometimes be managed simply, for example by removing the baby canine. Later on, treatment often combines a surgeon, who uncovers the tooth, and an orthodontist, who guides it into place. At Go Dentiste, screening is done at the exam and on X-rays: 514-533-3368.

02What to know

Understanding an impacted canine

Upper canines usually come in around ages 11 to 13; a delay isn’t always a problem, but it’s worth checking.

  1. Why it gets stuckLack of space, an abnormal eruption path, a baby canine that doesn’t fall out: after wisdom teeth, the upper canine is one of the teeth most often impacted.
  2. The role of X-raysThe position of the canine shows up on a panoramic X-ray. More precise imaging may be requested for planning.
  3. Early screeningAround age 10, the dentist checks whether the canine is heading to the right place. Removing the baby canine at the right time can sometimes help it come in.
  4. Combined treatmentIf the canine doesn’t come in, a surgeon can uncover it and bond an attachment to it, then an orthodontist guides it into place with braces.

Signs to watch for

  • A baby canine still in place after age 13 or 14
  • A bump on the palate or in the gum above a tooth
  • A canine that has come in on one side but not the other
  • A neighbouring tooth that is moving or tipping
  • A space that doesn’t close at the side of the smile

Good to know: An impacted canine usually doesn’t hurt. That’s why the X-rays taken at regular exams are important to spot it in time.

03Options

The options by age and position

The plan depends on the position of the canine and the available space.

  • Monitor and free up spaceIn children, removing the baby canine at the right time or creating space can sometimes let the canine come in on its own.
  • Surgeon and orthodontistThe canine is uncovered, then gently pulled into place with braces. We refer you to these specialists and coordinate your file.
  • Removing the canineIf the canine can’t be brought into place or threatens the neighbouring teeth, removing it may be discussed, along with a solution for the space.

04At our clinics

Screening at our clinics

At regular exams, our general dentists follow the eruption of the teeth and, if needed, take a panoramic X-ray to locate the canines. If a canine seems poorly oriented, they explain the options.

When the canine needs to be uncovered and guided, we refer you to an orthodontist and a surgeon. Your routine dental care continues in Valleyfield or Vaudreuil-Dorion.

06FAQ

Frequently asked questions: impacted canine

Is an impacted canine serious?

It isn’t dangerous in itself, but it can damage the roots of the neighbouring teeth or leave a space. That’s why it is screened for and monitored.

At what age should I be concerned?

A check around ages 10 to 12 is helpful. If the baby canine is still in place after age 13 or 14, an X-ray is recommended.

Is surgery always needed?

No. Caught early, the canine can sometimes come in on its own after the baby canine is removed. Later on, minor surgery to uncover it is often needed.

How long does treatment take?

Often more than a year, because the canine has to be moved slowly. The surgeon and orthodontist give you an estimated length.

Can an impacted canine be treated in adulthood?

It is sometimes possible, but more difficult. Depending on the position of the tooth, removal and another solution for the space may be discussed.
