01In brief

Upper teeth that stick out: do they need treatment?

Worth assessing, ideally early in children

When the upper teeth stick out well in front of the lower teeth, it’s called an overjet. Beyond appearance, front teeth that stick out a lot break more easily in a fall or from a blow, and the lips may have trouble covering them. Depending on the cause (position of the teeth, a habit like thumb-sucking, or a lower jaw that is set back), treatment ranges from braces or aligners to a referral to an orthodontist. Evaluation in Valleyfield or Vaudreuil-Dorion: 514-533-3368.

02What to know

What to know about overjet

The same visible result can have different causes, and the cause is what guides treatment.

  1. The teeth or the jaw?Sometimes only the teeth are tipped forward; sometimes the lower jaw is set back. X-rays help tell the difference.
  2. The role of habitsProlonged sucking of the thumb, a pacifier or the lip, and mouth breathing: these habits can push the teeth forward in children.
  3. A risk of injuryFront teeth that stick out a lot are more exposed to impacts. For an active child, a mouthguard is recommended during sports.
  4. The right time in childrenWhen growth is involved, an evaluation around ages 7 to 10 makes it possible to take advantage of jaw growth, often with an orthodontist.

Signs to watch for

  • The lips have trouble closing at rest
  • The upper teeth clearly stick out past the lower teeth
  • The chin looks set back in profile
  • Your child still sucks their thumb or lip
  • A front tooth has already been chipped in a fall

Good to know: If a protruding tooth has been hit or broken, it’s a dental emergency: call the clinic right away. In Valleyfield, emergencies are seen 7 days a week, subject to availability.

03Options

The options by cause

The plan is set after the exam and X-rays.

  • Tipped teeth onlyBraces or, for some moderate cases, aligners can bring the teeth back, often after creating space.
  • A habit is the causeThe first step is helping the child stop the habit; the position of the teeth may then improve on its own, before any appliance.
  • Set-back jawDuring growth, an appliance can guide the jaw, usually provided by an orthodontist. In adults, surgery may be discussed with an orthodontist.

04At our clinics

Overjet at our clinics

The dentist measures the overjet, looks at the profile and how the lips close, and takes the X-rays needed. The dentist explains whether the situation can be treated by our general dentists or whether an orthodontist’s opinion is preferable.

If your child plays sports, ask about a custom mouthguard: it protects protruding teeth until treatment.

06FAQ

Frequently asked questions: protruding upper teeth

Do “buck teeth” correct themselves?

Rarely. If a habit is the cause and it stops early, the position can improve. Otherwise, orthodontic treatment is usually needed.

Can an overjet be corrected with aligners?

For some mild to moderate cases, yes. A large overjet, especially one related to the jaw, often calls for braces or an orthodontist’s opinion.

Is an overjet dangerous?

It isn’t dangerous in itself, but protruding teeth break more easily on impact. A mouthguard is recommended during sports.

Should it be treated early in children?

Not always. Early treatment can be helpful if the overjet is large or if the child has already had an injury. An evaluation around age 7 helps decide.

And in adults?

The position of the teeth can be corrected with orthodontics. If the lower jaw is very set back, an approach combining orthodontics and surgery may be discussed with an orthodontist.
